COVID-19 Vaccine Update

As of January 4, 2021, individuals who work in dental practices of all types are eligible to receive the vaccine. This includes all dental staff.


The New York State Department of Health (NYSDOH) has set up a website for eligibility verification and obtaining appointments for dentists and their staff to receive the vaccine. Fill out the "Am I Eligible" survey at the link below to register for an appointment:

You will need to complete the NYS COVID-19 Vaccine Form in order to get vaccinated. NYS requires the provider administering the vaccine to check that you completed the form. In addition, you will need to bring proof of employment, such as an employee ID card, a letter from your employer or affiliated organization, or a recent pay stub.
